This crucial role involves working closely with the Warm Welcome team to assess needs and eliminate barriers for those facing homelessness. You will utilize your experience in social services and community health, ensuring participants gain access to shelter resources, healthcare, and social services. Building trusting relationships is key to facilitating lasting support for vulnerable individuals.

Key Responsibilities:

• Conduct direct outreach activities in East Portland

• Assist participants in accessing healthcare and shelter services

• Maintain participant charts with effective notes and outcomes

• Provide basic life skills education and support

• Adhere to privacy regulations and documentation standards

Requirements:

• High school diploma or GED required

• 1 year of experience in human services or community health

• Valid driver’s license and CPR certification

• Understanding of homelessness complexities

• Strong communication and relationship-building skills
