Certification Details
- Preference given to candidates available for a full schedule through the end of January and the first week of February.
- Day shift, 12-hour shifts.
- Start date:
1/17. - Position is Per Diem for 26 weeks.
Job Details
- Minimum compliance requirements must be met prior to starting; full compliance profile must be completed within two weeks after the start date.
- Active nursing license verification.
- Proof of MMR and Varicella vaccination or attestation of vaccination/declination.

Schedule Information
- Primary Source Verification of professional license and/or certification required within 30 days of start.
- Valid for 2 years:
Prime CA Sexual Harassment Prevention Training. - 2 COVID-19 vaccines and booster required or declination completed.
- Annual Respirator fit test required prior to start.
- Declinations not accepted for Hepatitis B, Influenza Vaccine, and Tdap at Shasta Regional Medical Center.
- Driver's license, State , or Passport required.
- Physical/Health Statement must be completed annually and state clear to work.
- National Criminal Background Check within one year of start date.
- Must score 80% or higher on Prime Core Competencies Exam and Prime Specialty Exam.
- Prime Skills Checklist does not expire on assignment.
- Relias accepted for Prime Color-blind test, must be completed annually.
- Prime Agency Attestation and Prime SAM/EPLS required.
- Prime Form I-9 (Employment eligibility verification) required.
- Upload Essentials Form and Post Test found in Document Library.
- Copy of Diploma or Transcripts accepted if third party verification is not available.
- Workplace Violence Prevention training required.
- Prime Drug Screening (minimum 10-panel test) within 60 days of start, including THC saliva test for CA locations.
- Resume must be up to date and reflect accurate recent work history.
- Two references required, one from a manager or direct supervisor.
- Notarized name affidavit required for any name discrepancies between credentials.
