Home Health LPN/LVN

We are seeking a dedicated Home Health LPN/LVN to provide high-quality patient care in the community. This role focuses on delivering routine home health visits to patients with various medical needs.

• Provide direct patient care within the home health setting.
• Assess and monitor patient health status, including wound care, ostomies, and post-surgical wounds.
• Manage care for patients with cardiac issues, chronic heart failure (CHF), pneumonia, and IV antibiotics with PICC line management.
• Complete routine visits based on a point system: up to 7 patients per day.
• Utilize electronic health records through Home Care Home Base.

• Must possess a valid BLS Certification.
• Must have a valid Driver's License and Insurance.
• Minimum of 2 years of relevant experience in home health settings.
• Proficiency in wound care, wound vacs, and ostomy care is required.

• Comfortable clothing is acceptable; scrubs are not required.
• Orientation will be provided, and you will have a preceptor assigned for support.
• No weekends, holidays, or on-call requirements.
• This position does not involve patient scheduling or case management.

The position is based in a rural community that offers a variety of recreational opportunities, including access to mountains, lakes, and wilderness areas. The area is approximately a four-hour drive north of San Francisco and two hours north of Sacramento.
