We are looking for a proactive and highly organised Pharmacy Care Coordinator to join the Nightingales Primary Care Network Pharmacy Team in Redditch. This role is for up to 35 hours a week but hours can be discussed at interview.

This pivotal role will provide administrative, operational and coordination support across our six-member practices, helping to deliver high-quality pharmacy services, prescribing initiatives and quality improvement programmes. Working closely with Clinical Pharmacists, Pharmacy Technicians, GPs and Practice Managers, the successful candidate will play a key role in ensuring effective communication, data management, service delivery and patient-focused improvements across the network.

Main duties of the job
  • Provide administrative and operational support to the PCN Pharmacy Team.
  • Coordinate pharmacy staff rotas, location schedules and weekly timesheets.
  • Create and manage EMIS searches, templates and workflows to support pharmacy services.
  • Set up and maintain Accu Rx templates and digital workflows.
  • Organise and support pharmacy meetings, training sessions and multidisciplinary team meetings.
  • Prepare agendas, take meeting minutes and monitor action plans.
  • Maintain accurate spreadsheets, reports and data submissions to support contractual requirements and prescribing initiatives.
  • Support the delivery of quality prescribing and medicines optimisation programmes.
  • Analyse and collate service and prescribing data to support performance monitoring and improvement projects.
  • Act as a key point of contact between the Pharmacy Team, GP practices, PCN colleagues and external healthcare organisations.
  • Support governance, reporting and quality improvement activities across the PCN.
  • Assist with pharmacy appointment management and service delivery across the network.

Nightingales Primary Care Network

Nightingales PCN is a collaboration of 6 GP practices in Redditch extending into Wythall with a shared population of 44,194. Nightingales PCN has a shared ambition to innovate general practice and build a sustainable model for general practice for the future.

For new PCN staff, an induction and mentorship approach will be introduced which secures the opportunity to build relationships across the practices and creates effective development and supervision opportunities in order to build necessary expertise and experience.

The Care Coordinator will possess excellent communication, administration and organisational skills working directly with the PCN Pharmacy Team, PCN Management Team and the clinicians across the PCN.

Job Purpose

The Pharmacy Care Coordinator role provides dedicated administrative and operational support to the PCN Pharmacy Team and associated GP surgeries. The post-holder will play a key role in supporting delivery of pharmacy services, prescribing initiatives, contractual requirements and wider PCN priorities.

Working closely with Clinical Pharmacists, Pharmacy Technicians, GPs, Practice Managers, PCN colleagues and external partners, the post-holder will support coordination of pharmacy work streams, ensuring accurate data management, reporting, communication and delivery of services.

Core Responsibilities

Pharmacy Team Coordination and Operational Support

Provide administrative and operational support to the Pharmacy Team across the PCN and associated GP surgeries. Coordinate weekly staff timesheets and manage pharmacy team location schedules. Support communication between pharmacy staff, GP surgeries, Practice Managers and PCN teams. Organise Pharmacy Team meetings, prepare agendas, record minutes and track actions. Attend Practice Manager, PCN Board and wider multidisciplinary healthcare professional meetings.

EMIS, Digital Systems and Workflow Support

Create, run and manage EMIS searches to support clinical and administrative work streams. Design, develop and maintain EMIS templates to improve pharmacy processes and data capture. Create, set up and manage Accu Rx templates and workflows. Support achievement of the Neighbourhood Health Delivery Plan and Quality Prescribing Medicines Scheme contract.

Oversee pharmacy appointment slot management on EMIS.

Support implementation and management of Proxy Access within care homes.

Meetings, Training and Communication

Coordinate pharmacy meetings, MDTs and training sessions. Assist with delivery of GP surgery training sessions in person or via Microsoft Teams. Liaise with external agencies to arrange training provision. Maintain communication regarding work streams, reporting requirements and submission timelines.

Data Management, Reporting and Contract Support
