Are you an experienced SEND professional looking to lead and shape outstanding transition support for learners with additional needs?

We are seeking a Transition Practice Lead to develop and oversee a high-quality transition offer for learners with EHCPs, SEND and other learning barriers. Reporting to the Head of Inclusion, you will lead transition planning, EHCP consultation processes and a team of Transition Officers, ensuring learners receive the right support from day one.

As Transition Practice Lead, You Will
  • Lead transition, assessment and admissions processes for learners with SEND.
  • Coordinate and quality assure EHCP consultations and statutory processes.
  • Develop effective support plans and transition arrangements.
  • Line manage and support Transition Officers.
  • Build strong partnerships with Local Authorities, schools, SENCOs, parents and carers.
  • Monitor provision, learner needs and High Needs funding requirements.
  • Drive high-quality, inclusive practice across the college
The Successful Candidate Must
  • Have a relevant qualification in SEND (Learning Needs), education, wellbeing or in a related area or willing to work towards one
  • Knowledge and training relevant to SEND transition, EHCP processes or inclusive practice
  • Experience of line management
  • Knowledge and understanding of the SEND Code of Practice, particularly in relation to FE transition and EHCP processes
  • Have experience working with young people in an educational or support setting.
  • Strong communication, organisation and relationship-building skills.
  • Understanding of safeguarding and inclusive practice.
  • Level 2 English and Maths (or willingness to achieve)
Interviews are expected to take place from 20th of August onwards.
