Community Staff Nurse

The post holder will work under the direction of the Responsive Services Charge Nurse/Sister as part of the Responsive Services team which consists of Intermediate Care Team, Urgent Community Response team, Referral Hub and the Virtual Ward. The post holder will work in collaboration with Primary Care and Secondary Care Colleagues, Community Nursing and other Specialist Health Care Teams across First Community, statutory and voluntary agencies to deliver excellent standards of patient care, assessing and treating patients who are acutely unwell and requiring support to avoid admission to the acute hospital or those who have recently been discharged home.
