Support Worker - Home from Hospital

Location:

Redhill, Surrey with travel around the area

Hours:

21 Salary: £24,479 per annum pro-rata (£13.45 per hour)

Contract type:
Fixed Term Contract Until March 2027

Driving Requirement: A full UK driving license held for a minimum of 2 years is required and access to a vehicle. Business mileage reimbursed.

- Home from Hospital in Surrey. This is an excellent opportunity to contribute to the well-being and independence of individuals within our community.

What will a day in the life of a Support Worker involve?
  • Managing a caseload to provide emotional and practical support for our service users who have been discharged from hospital including welfare calls, shopping, and home visits.
  • Supporting adults after a hospital stay to help them recover safely at home.
  • Responding to referrals from clinicians and community health professionals.
  • Assessing needs and following a personalised support plan for each service user.
To be a successful Support Worker, what will you need?
  • A kind, calm and confident approach with great people skills.
  • The ability to work independently and as part of a team.
  • A full UK driving licence and access to a vehicle
  • Basic IT skills and a commitment to following procedures.
