Key Responsibilities
  • Provides orientation to new patients to the program’s rules, policies, procedures, and patients' rights.
  • Conducts Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by federal and state regulation, CARF guidelines and Pinnacle Policies.
  • Involves patients in the assessment, treatment planning, and intervention aspects of treatment, giving patients a sense of entitlement to their own recovery.
  • Administers the biopsychosocial assessment, evaluating all areas of patients' lives, including addictions, medical and mental health history, socioeconomics, legal matters, etc., at intake and on an ongoing basis.
  • Recognizes and utilizes patients’ Strengths, Needs, Ability, and Preferences (SNAP) to develop and implement individualized Treatment Plans that effectively help patients meet short‑ and long‑term goals.
  • Determines the clinical necessity of counseling services based on the assessment and evaluation of the patients.
  • Monitors closely and documents any changes in circumstances with patients, especially those that may affect recovery, identifying relapse indicators and promoting relapse‑prevention interventions early.
  • Complies with and implements the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
    • Evaluates patients for high‑risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, suicidal or homicidal tendencies).
    • Administers the BAM‑R, C‑SSRS, GAD‑7, and PHQ‑9 screening tools to determine severity of mental/emotional status.
    • Evaluates risk factors for each case, considering appropriateness and professionalism in treatment modalities.
    • Recommends immediate referral to specialists when needed and obtains waivers for release of information.
    • Flags high‑risk patients to Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL).
    • Refers patients to Medical Directors or RCLs based on urgency, following informal or scheduled referral policies.
    • Reviews physician notes to confirm addressing of high‑risk factors and maintains communication with supervising staff.
    • Documents and circulates minutes from treatment team/case conference sessions.
  • Provides professional counseling and referral services by:
    • Maintaining referrals to specialized services and community resources.
    • Providing individual and group counseling clinically as needed; addressing issues identified in assessments.
    • Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy and Motivational Interviewing.
    • Scheduling regular counseling sessions, issuing appointment cards, and recording appointments in the EHR scheduler.
    • Documenting counseling entries with signature and date as required by regulations and guidelines.
    • Engaging in community relations and outreach efforts.
    • Attending all recommended trainings and case conferences.
