Summary:

The Rehabilitation Department provides therapy and nursing services to patients, families and patient’s caregivers including:
Rehabilitation Nursing Care, Physical Therapy, Occupational Therapy and Speech Language Pathology. Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.

Position Overview

Evaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient’s abilities and goals. This position does not directly manage any other caregivers.

Essential Functions and Duties
  • Adheres to Oregon State Speech-Language Licensing Board Standards.
  • Performs evaluation and treatment of patients with speech, hearing, language, voice, cognitive or swallowing disorders.
  • Provides community resource, knowledge and referral services.
  • Provides staff consultation and training.
  • May be required to drive Rehabilitation van and meet all related SCHS vehicle policy requirements.
  • Functions as a member of the Interdisciplinary Care Team to assure care is accomplished effectively and efficiently in a cost effective manner.
  • Participates in clinical care conferencing.
  • Gathers data and accurately documents information in a timely manner demonstrating compliance with department standards.
  • Participates with team members in facilitating patient’s and family’s learning throughout the course of care. Reinforces patient’s continued health care through teaching and/or referral to community agency follow-up.
  • Supports the vision, mission and values of the organization in all respects.

  • Provides and maintains a safe environment for caregivers, patients and guests.
  • Conducts all activities with the highest standards of professionalism and confidentiality.

  • Delivers customer service and/or patient care in a manner that promotes goodwill, is timely, efficient and accurate.
  • May perform additional duties of similar complexity within the organization, as required or assigned.
Education

Required: Masters degree in Speech-Language Pathology from a college or university accredited by the American Speech-Language Pathology & Hearing Association.

Preferred: N/A

Licensure / Certification / Registration

Required: Must have necessary requisites for licensure as a Speech‑Language Pathologist by the Oregon State Board of Health. Must have active Certificate of Clinical Competence (CCC) from ASHA. Must have a valid Oregon driver’s license and meet St. Charles Health System driving requirements. Ability to travel to business functions/trainings/meetings and all St. Charles Health System worksites. Current AHA Basic Life Support for Healthcare Provider certification.

Preferred: N/A

Experience

Required: 1 year experience in therapy procedures with medical based diagnoses and familiarity with rehabilitation practices.

Preferred: Previous experience in a hospital-based setting.
