The professional Registered Nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. The RN may delegate appropriate tasks to direct patient care staff including RNs, LVNs/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health‑care team, the RN participates in decision‑making, teaching, leadership functions, and quality‑improvement activities that enhance patient‑care outcomes and facility operations.
PrincipalDuties and Responsibilities
- Perform essential nursing functions under the direction of the Supervisor and in collaboration with other registered nurses.
- Collect and analyze patient data throughout hemodialysis treatment, document in the medical record, and adjust treatment as indicated.
- Assess, collaborate, and document patient/family learning needs; provide initial and ongoing education.
- Direct and provide safe delivery of hemodialysis therapy to assigned patients.
- Administer medications as prescribed or per approved algorithm; document justification and effectiveness.
- Initiate or assist with emergency response measures.
- Serve as resource for health‑care team; participate in staff training and orientation as assigned.
- Ensure correct laboratory collection, processing, and shipping; reschedule missed or insufficient collections.
- Identify expected outcomes; document and update the nursing assessment and plan of care through collaboration with the interdisciplinary team.
- Ensure patient awareness related to transplant and treatment‑modality options.
- Complete CAP requirements to advance.
- Perform all other duties as assigned by Supervisor.
- Provide direct patient care involving heavy lifting, patient movement, and ambulation; assistance may be available.
- Require frequent, prolonged periods of standing and ability to bend over.
- Occasional movement of machines and equipment up to 200 lbs.; lift up to 30 lbs. as high as 5 feet.
- Work environment is a health‑care facility with controlled temperature and moderate noise.
- Exposure to infectious and contagious diseases/materials may occur.
- Daily work includes desk work, computer work, patient interaction, and collaboration with staff.
- Travel to training sites or other facilities may be required.
- Essential functions may be performed at other locations with similar demands.
Oversight of Patient Care Technicians, LPNs, LVNs, and RNs as a Team Leader or designated Nurse in Charge, after meeting the following criteria:
- Successful completion of all FKC education and training requirements for new employees.
- Minimum 9 months experience as an RN.
- Graduate of an accredited School of Nursing.
- Current state licensure.
- Current or successful completion of CPR BLS Certification.
- Must meet practice requirements in the state in which employed.
- Entry‑level RN with less than 2 years of nephrology nursing experience.
- Chronic/acute hemodialysis experience is preferred.
