Work Schedule:
Full Time, 40 hours a week, Typically Monday – Friday 8:30am – 5pm or 9:30am-5:30pm(flex). The initial 2-week training is in-person at a Hopelink worksite. After training, mostly in-person work (typically, 3 days onsite and 1-2 day remote). Some flexibility on determining schedule. Case Managers may need to provide client coverage at other Hopelink sites.

The career opportunity :
Hopelink provides vital resources to people experiencing financial and housing instability in our community. The Case Manager provides assessment, advocacy, referrals, and other support services to low‑income families in shelter, transitional, non-time limited housing programs as well as community-based settings. The primary goal is to help families through their own resilience to make positive life changes and substantial progress towards self‑sufficiency, with a focus on increasing income & obtaining and maintaining permanent housing.
The ideal candidate will be flexible, compassionate, excited to learn, and is a team player who works well in a collaborative team environment alongside community partners. A combination of on‑site and remote options is available for this role after training period is complete and job proficiency is demonstrated. Case Managers may need to provide client coverage at other Hopelink sites.

- Experience working with diverse communities, preferably low‑income, older adults, people with disabilities, immigrants, refugees, youth, and/or veterans.
- Two years’ experience in social services or a community-based organization; in housing case management preferred.
- Knowledge of subsidized housing resources and housing system to address family homelessness preferred.
- Computer skills required (Word, Excel, Internet). Office 365 and Teams, zoom, SharePoint, databases.
- Introductory knowledge of human services client assessment methods, principles, and techniques.
- Able to work independently and on a team.
- Excellent communication skills (verbal and written).
- Reliable personal transportation, a valid driver’s license and proof of auto liability insurance required.

Examples of what you will be doing:
- Manage a moderate size case load of around 15‑20 families.
- Interview and determine the needs of families and individuals in various life domains.
- Develop case management case plans in collaboration with families and individuals.
- Assist families and individuals with identification and achievement of goals including addressing barriers to success.
- Make appropriate referrals and coordinates services with community agencies based on clients' needs and interests.
- Ensure service interactions at all levels are done through a trauma informed perspective, paid training for cutting edge Trauma informed approach training.
