Position: Home Health Field Registered Nurse (RN)

Position Overview

Entry‑level Field Registered Nurse position designed to develop fundamental nursing skills within a home health setting while gaining an understanding of the company's operational goals and efficiencies. Serves as a foundational role for registered nurses aspiring to advance into case management. Works under a Case Manager Registered Nurse.

Required schedule:
Tuesday‑Saturday or Sunday‑Thursday. All opportunities require field visits to patients’ homes.

Duties and Responsibilities
  • Providing hands‑on patient care including PICC lines, wound vac & care, kangaroo & feeding pumps, NG/G/J tube management, staple & suture removal, drainage systems, Foley/suprapubic catheters, etc.
  • Provides direct patient care as defined in the state Nurse Practice Act.
  • Implements a plan of care initiated and directed by the case‑management RN.
  • Provides accurate and timely documentation within 24–48 hrs.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Assists the registered nurse in performing skilled needs procedures and duties, preparing equipment and materials and teaching self‑care techniques.
  • Participates in coordination of home health services, reporting needs for other disciplines.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the case‑management RN.
  • Retrieves communication on assigned patients; uses information to establish priorities of care.
  • Records pertinent and concise information that reflects patient needs, problems, capabilities, and limitations.
  • Records and reports any incident that has a bearing on the patient.
  • Attends weekly case conferences as required.
  • Ensures patient privacy in maintaining medical records.
  • Maintains professionalism and seeks opportunities to enhance clinical knowledge.
  • Maintains productivity standards as directed.
  • Completes and reviews start‑of‑care assessments, documentation, and plans of care with the case manager.
  • Participates in on‑call duties as required by weekend rotation.
Job Specifications
  • One year recent nursing experience (internal candidates may be exempt).
  • Preferred: one year of home health experience.
  • Experience with high‑acuity patients is a plus.
  • Experience working with an interdisciplinary team is highly desired.
  • Graduate of an accredited nursing school (Bachelor's degree).
  • Current CA RN license.
  • Current Basic Life Support (BLS) card.
  • Valid CA driver’s license with acceptable record.
  • All licenses and certifications must be current at hire and throughout employment.
  • Self‑directed, critical thinker, outstanding interpersonal skills, organized, computer literate; excellent observation, verbal and written communication, problem‑solving, basic math; nursing skills per competency checklist.
  • Must maintain current compliance training and certifications annually.

Compensation

Estimated wage range: $130,000 – $160,000 per year, based on experience above the minimum requirements. Compensation follows a pay‑per‑visit model. Benefits include comprehensive health coverage and a 401(k) plan with up to a 4% match.
