Responsibilities
The Transitions of Care Coordinator is responsible for coordinating patient flow from hospital to outpatient services to include but not limited to outpatient medical/surgical follow up and testing. Functions as a liaison, patient advocate and navigator between hospital and outpatient services, Fosters and delivers customer services and patient access.
The Transitions of Care Assistant Supervisor is responsible for daily oversight of the transition of care process and is expected to:
Coordinate the patient flow between hospital services and outpatient services to ensure continuation of care.
Maintain prompt, clear communication with the patient and providers to ensure timely follow up.
Assist the patient as needed with navigation of health care services and systems.
Function as liaison between services to assist the patient with timely care.
Maintain current knowledge of insurance requirements to ensure patients are provided with accessible services.
Provide reports monthly by the 10th of each month. Quarterly and annual summaries required.
Ensures accommodation of individualized interpretive language services as needed.
Demonstrates flexibility in work schedule and assignments.
Maintains a respectful and positive working relationship with patients, providers, co-workers, other departments, and insurance companies.
Assists in special projects and other duties as assigned.
Abides by all HIPAA policies and procedures pertaining to Patient Health Information. Adheres to St. John's Riverside Hospital attendance policy. Regular attendance and timelines are required. CUSTOMER SERVICE EXCELLENCE
Provide customer service in accordance with practice Mission of St. John's Riverside Hospital and the St. John's Medical Group.
Demonstrate ability to be courteous and respectful when interacting with patients, patient's significant others/caregivers, co-workers, and all Other individuals.
Maintain strict patient confidentiality in accordance with policy and procedure and HIPAA requirements.
Enact proper chain of command for patient complaints, office and practice needs that require management input and awareness.
Qualifications
Bachelor's degree in related field preferred.
Advanced knowledge of MS Office products including Word, Excel, PowerPoint & Outlook
Excellent written and oral communication skills, Excellent telephone and interviewing skills.
Exceptional attention to detail and accuracy.
Experience in customer service in medical setting required.
Travel between locations required. Bi-lingual preferred.
