Join our team as a Licensed Practical Nurse (LPN)! The LPN is part of a team of LPNs who are responsible for providing nursing care within their scope of practice to participants within the Community Living Support programs of the agency. Reporting to the Manager of Clinical and Community Services, the LPN acts as a liaison between participants and the person-centered support staff, promotes health and healing by functioning as a member of the care team to provide optimum care for individuals with complex medical health needs.

As part of an interdisciplinary team the LPN carries out their responsibilities in accordance with statutory and regulatory standards, agency policy and within the established LPN scope of practice.

Duties and Responsibilities
  • Provide LPN services, within defined scope of practice, to participants based on participants assessment and care planning procedures.
  • Acts as the communication and coordinating link for participants consulting with a variety of practitioners while collaborating and functioning as part of the program team.
  • Co-ordinate individual appointments and referrals.
  • Ability to communicate effectively through both written and verbal, nurturing excellent human relations with individuals, families, and other team members.
  • Communicates with other members of the team to provide continuity of participant care and promote collaborative efforts directed towards quality participant care. Admitting new participants, recording their medical history, and monitoring their care.
  • Provides support, information, psychoeducation, and follow-up to individuals.
  • Ability to work with minimal supervision and to evaluate and meet individual care needs, demonstrating initiative and problem‑solving skills.
