Primary Health Care Nurse Practitioner

Location: Carlyle

Elevate community health as a Nurse Practitioner providing care in Redvers and surrounding areas. This role blends health assessment, disease prevention, and collaboration with diverse populations.

You will report to a designated supervisor while delivering essential primary health care services. Supporting urban residents, rural communities, and priority groups like newcomers and teens, this position may also expand to nurse-led extended-hour clinics. Engage in quality improvement, patient safety initiatives, and community development projects.

Key Responsibilities:

• Conduct comprehensive health assessments for assigned populations

• Promote health education and disease prevention strategies

• Collaborate with physicians and multidisciplinary care teams

• Coordinate referrals and ensure effective patient care transitions

• Participate in quality improvement and evaluation activities

Requirements:

• Master of Nursing – Nurse Practitioner (MN-NP)

• Registration with the College of Registered Nurses of Saskatchewan

• Valid Class 5 driver’s license

• Basic and Advanced Life Support Certifications

• Knowledge in community development and advanced computer skills
