In this role, you will be instrumental in managing diabetes care, including conducting assessments, educating patients, and providing continuous monitoring. Key Responsibilities:
• Collaborate with physicians and care teams on protocols
• Assess patient health status and lifestyle factors
• Educate patients on diabetes management techniques
• Monitor chronic disease indicators for patient progress
• Support community resource referrals as needed
Requirements:
• Registered with the College of Registered Nurses of Saskatchewan
• Bachelor of Science in Nursing or equivalent
• Minimum of 2 years experience in nursing
• Preferably a Certified Diabetes Educator
• Proficiency with EMR systems required
