In this role, you will hold a pivotal position as a Clinical Coordinator for Palliative Care, ensuring client and family-centered service in accordance with SRBA standards. Your expertise will be essential in providing assessments, determining eligibility for home care programs, and facilitating client access to palliative services. Strong decision-making and critical thinking skills will be paramount in coordinating care with multidisciplinary teams for various phases of client illnesses.
Key Responsibilities:
• Assess client needs for access to palliative care
• Determine eligibility for home care services
• Facilitate discussions with clinicians regarding client care
• Manage case coordination and service referrals
• Promote best practices in patient care through consultation
Requirements:
• Bachelor of Science in Nursing (BScN)
• Canadian Hospice Palliative Care Nurse Certificate (CHPCN)
• Minimum of three years in long-term care nursing
• Experience working in a multidisciplinary team
• Licensed with the College of Registered Nurses of Saskatchewan
