Position Purpose Summary
The role is focused on helping individuals find solutions to problems of social adjustment that are difficult for individuals to navigate on their own.

Accountabilities

Case Management:

Utilize best practices in professional collaboration to work with partners across Canada including law enforcement, children’s aid, Indigenous organisations, primary and emergent healthcare.

Develop case plans, utilise case‑management tools, report case notes and develop strategies for clients within a caseload, providing case management, case coordination and referrals.

Identify and complete paperwork for various applications.

Provide home visits and in‑home training for various program participants as required.

Support relevant volunteers and co‑workers in implementing programming across multiple sites and provide input to program design.

Provide consultation support to community agencies and refer to training opportunities.

Assess client suitability to internal and external programmes and agencies as required.

Ascertain and provide participants with referral services within the community and/or social services resources.

Ensure client supervision and safety in designated programmes and comply with relevant policies and procedures.

General Program Support:

Contribute and provide recommendations on the development of new and existing programmes for clients, assist with statistics collection and measure tangible outcomes.

Use knowledge of issues pertaining to the scope of programmes to contribute to the development of programme materials and partnerships.

Contribute to the development of administrative frameworks and best practices for programme delivery, including new and innovative approaches to programming, community involvement, crisis intervention and cross‑industry collaboration.

Ensure that programme procedures are maintained in one's own work and, when providing recommendations to others, support the Program Manager in developing relevant policies and present to the Leadership team, contributing to the drafting of learning guides and facilitator materials where required.

Provide coordination and administrative assistance in the delivery of designated programmes, follow outcome matrices to attain reportable and significant outcomes, complete monthly reports and end‑of‑project reports.

Seek community donations and in‑kind support for minor programming items, develop relationships with external stakeholders, participate in community‑based events and represent the organisation within the scope of duties per Leadership request.

Assist the management team in establishing meaningful connections with community partners, support and participate in stakeholder evaluations and focus groups, attend off‑site meetings with stakeholders as required, including serving as an active member on community committees.

Maintain appropriate records and statistics, ensuring all required documentation is accurate and complete.

Professional Development:

Participate in training, meetings, case‑management development, administrative system orientations and other required activities.

Participate in test‑run programming and utilise provided learning‑lab techniques to refine project concepts, materials and delivery.

Model positive behaviours and boundaries with clients and contribute to the mission and goals of The Salvation Army.

Perform other work‑related duties as assigned.

Critical Relationship Management

Internal: MU leaders and staff, clients and their family members, volunteers.

External:
Community members, community groups/organisations, police, agencies.

Managerial/Technical Responsibilities
Reports directly to the Housing Service Manager. No direct reports. Responsible for workflow maintenance and oversight.

May provide general coordination and guidance to non‑supervisory staff and volunteers within the scope of one's own work without formal authority.

Financial and Material Management

Minor material responsibilities (access to confidential information).

Safekeeping of materials and equipment for own use.

No financial responsibilities.

Working Conditions

Typically an office environment.
