As a permanent member of our team, you will handle a caseload of 8-10 clients, delivering essential case management services.
Your role includes assisting clients with appointments, housing, and financial management while collaborating with a multidisciplinary team for effective care coordination.
Key Responsibilities:
• Provide direct management of complex client cases
• Assist clients in accessing healthcare and housing
• Coordinate care with the multidisciplinary team
• Employ harm-reduction and crisis-management techniques
• Tailor individualized service plans with client input
Requirements:
• Valid driver’s license with a reliable vehicle
• Current First Aid/CPR certification
• Excellent teamwork and leadership skills
• Strong capabilities in crisis management
• Knowledge of trauma-informed care practices
