PIPS-Enhanced Intensive Case Manager
Role Overview
We are hiring a full‑time, permanent social worker or occupational therapist to serve a small case load (8‑10) of complex mental health clients, many with co‑occurring substance use disorders.
Responsibilities
Provide direct case management, including assisting clients with physical and mental health appointments, housing, social/rec programming, financial management, and other services as needed.
Coordinate care with a multidisciplinary team that includes a team lead, other ICMs, and support workers.
Apply crisis‑management techniques, harm‑reduction principles, trauma‑informed care, and person‑centered philosophies.
Collaborate with clients to develop and monitor individualized service plans.
Qualifications
Valid driver’s license and ownership of a reliable vehicle.
Current First Aid/CPR certification.
Excellent teamwork and leadership skills.
Strong crisis‑management skills.
Knowledge of harm‑reduction, trauma‑informed, and person‑centered philosophies.
Must be able to become a registered social worker or occupational therapist; registration cost will be covered by Phoenix.
Schedule
Monday‑Friday, four day shifts and one evening shift (1:00 pm‑9:30 pm).
