Case Manager

Position: Full-Time Case Manager
Join Phoenix Residential Society as a full-time Intensive Case Manager in Regina. Your skills in crisis management and harm reduction will be pivotal in supporting clients facing complex challenges.

In this temporary role until November 13, 2027, you will serve as part of the Wahkohtowin Program, managing a case load of 12 individuals with co-occurring disorders. Working collaboratively with a supportive team, your responsibilities will include guiding clients through health services, securing stable housing, and executing social programming. Candidates with a deep understanding of trauma-informed practices and strong leadership abilities are invited to apply.

Key Responsibilities:

• Manage cases for 12 clients with complex needs

• Assist clients with health and housing support

• Facilitate recreational and social activities

• Guide clients in financial management practices

• Work alongside a diverse team to ensure integrated care

Requirements:

• Bachelor’s Degree in a related discipline required

• 1-2 years of experience in a similar setting

• Valid Saskatchewan Class 5 driver's license needed

• First Aid/CPR certificate required

• Must have the potential for social worker registration
