The Role

As part of NHS Greater Glasgow & Clyde and provided within Renfrewshire Learning Disability family of services is a joint health and social care specialist team for adults with learning disabilities. The post holder will work alongside the wider learning disability nursing and MDT that contributes to the priorities within this service to develop and maintain partnerships between Specialist Learning Disability In-Patient Services, NHS Acute Care, GP Practices and voluntary agencies to support people to facilitate access to appropriate services.

Key Responsibilities
  • Provide highly specialist holistic assessments, including risk assessment, care planning, implementation and evaluation for a complex caseload, while overseeing nursing staff caseloads.
  • Deliver, review and monitor nursing care that supports mental and physical wellbeing, addressing issues such as vulnerability, gender, domestic abuse, adult/child protection and self-harm.
  • Ensure therapeutic staff-patient relationships meet complex needs within a framework of equality, dignity, respect and cultural/spiritual sensitivity.
  • Ensure nursing staff work within competency frameworks, delivering evidence-based interventions, psychological therapies and Positive Behaviour Support.
  • Promote values-based, person-centred care through partnership working with service users and carers.
  • Ensure professional practice complies with the NMC Code and relevant legislation (Mental Health Act, AWI, ASP).
  • Lead the nursing team in maintaining strong clinical governance, including communication, leadership, policies, audits, evidence-based practice, learning culture and risk-reduction strategies.
  • Provide regular caseload, line management and clinical supervision, ensuring systems are in place for consistent supervision across the team.
  • Monitor nursing practice and performance, ensuring systems support ongoing professional and practice development.
  • Provide specialist nursing advice to the nursing team, Learning Disability Service and wider health and social care partners.
  • Ensure safe and effective transitions of care, with information shared appropriately under legislative and policy frameworks.
  • Manage delegated budgets and resources within the area of responsibility.
  • Work with Practice Education Facilitators and academic partners to support and quality-assure the service as a learning environment.
  • Liaise with referrers, interface services and statutory/non-statutory agencies involved in patient care across hospital and community settings.
  • Support the Operational Manager in developing the Learning Disability Service and address team issues in their absence.
  • Produce formal reports and contribute to Learning Disability Service performance indicators.
  • Lead improvements to care pathways, reduce barriers and enhance health outcomes.
Qualifications
  • 1st Level Registered Nurse
  • Post graduate qualification in appropriate discipline at Masters level / working towards a Masters level education or equivalent level of experience
Experience
  • Experience of working with people with Learning Disability.
  • Evidence of continuing professional development.
  • Understanding of wider service issues, connections and priorities.
  • Proven ability to work effectively in partnership.
  • IT skills e.g. word processing, email, internet.
  • Excellent communication skills (both verbal and written).
  • Ability to work to challenging deadlines.
  • Ability to work on own initiative.
  • Leadership and management skills.
  • Problem solving skills.
  • Ability to work as part of a multi-professional, multi-agency virtual team.
  • Experienced in inter-professional assessment.
  • Ability to motivate others.
