Responsibilities
  • Coordinates and reviews flow of files and documentation in Department. Actively participates in Case Management and Treatment Team meetings.
  • Responsible for reviewing patient charts to assess whether criteria for admission and continuation of treatment has been documented; gathers data and responds to request for records from fiscal intermediary; gathers clinical and fiscal information and communicates status of open and closed accounts for multiple levels of Case Management reporting.
  • Able to work independently and use sound judgment. Knowledge of Federal, State, and intermediary guidelines related to inpatient, acute care hospitalization, as well as lower levels of care.
  • Coordinates communication regarding discharge referrals as requested by clinical staff, fiscal intermediary, patients and families.
Qualifications
  • Associate or Bachelor’s in healthcare or related field or equivalent work experience in the related field.
  • Acute care work experience preferred.
  • Intermediate computer skills including data entry.
  • Proficient communications skills both written and oral.
