Position: Social Work Care Coordinator 1-OP

Position Purpose

This position is responsible under the supervision of the department manager for providing patients and families with emotional support, advocacy as appropriate, financial information, transitional care planning, and social work services. This position also serves as a liaison between the medical staff and the patient/family and provides interventions designed to minimize patient stress and maximize coping skills. This position requires the ability to advocate on behalf of the client to ensure their connection to necessary resources for their continued success.

Nature

and Scope

This position provides psychosocial services to patients, families, and consultation to staff to assist in dealing with barriers to patient self-management. In addition to working as an integral member of the patient’s interdisciplinary care team, this position also provides condition‑specific educational information, resources, and support to assist patients, families, and caregivers. The incumbent may work with individuals at different stages of the life span ranging from early childhood to late adulthood and must provide optimal patient care.

An ability to understand and apply best practices related to conditions experienced by patients of the assigned population(s) is critical. The major challenge to this position is the ability to prioritize and assess critical situations while maintaining quality services under stressful conditions. The position has the authority to review and evaluate each case and establish care‑plan priorities. For some care management programs/departments this position may be required to work with patients in the community outside of a Renown facility yet remain under Renown’s stewardship.

Contact with these patients may be by phone, in‑person during an appointment, or via a home visit.

Knowledge, Skills & Abilities
  • Knowledge of social work skills, theories, and intervention.
  • Ability to complete psychosocial assessment and develop and implement care plans specific to the patient’s unique needs and conditions.
  • Requires knowledge and understanding of medical terms.
  • Ability to work effectively as a collaborative member of an interdisciplinary team, and to facilitate close working relationships and cooperation within the Health System and community.
  • Ability to apply independent and mature judgment and establish priorities.
  • Ability to interact effectively with persons of differing ethnic and socio‑economic backgrounds.
  • Demonstrate the knowledge and skills necessary to provide care, based upon the physical, motor, sensory, psychosocial, and safety needs and development, to the age of the patient.
  • Skills to assist patients, families, and caregivers in coping with crisis situations.
  • Knowledge of community resources available to meet the psychosocial needs of patients.
  • Ability to perform initial assessments on referred patients within time frames determined by the department.
  • demonstrate creative problem solving and critical thinking.
  • Provides patients and caregivers the knowledge,, and support necessary to empower them to be successful with self‑management.

Minimum Qualifications Education

Must have working‑level knowledge of the English language, including reading, writing, and speaking English. Appropriate education to obtain and maintain Social Work licensure in the State of Nevada.

Experience

Requires one‑year experience in the social service field, or completion of an internship program in a healthcare setting or within a department‑specific focus. Applicants with experience in health care settings preferred.

License(s)
  • Ability to obtain and maintain State of Nevada Social Work licensure.
  • Community Care Management (200741, 530346):
    Valid State of Nevada or California driver’s license and ability to pass Renown Health’s Department of Motor Vehicles Report criteria and current automobile insurance.
  • Required for this position:
    Fingerprints must be able to pass Nevada Division of Public and Behavioral Health (DBPH) background checks upon hire and every 5 years per State of Nevada Revised Statute (NRS 449.123) to remain in this position.

Computer / Typing

Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel, and Word and have the ability to use the computer to complete online learning requirements for job‑specific competencies, access online forms and policies, complete online benefits enrollment, etc.
