Position Purpose

This position provides face‑to‑face, virtual, or telephonic care. The Care Coordinator collaborates with both clinical and non‑clinical team members, coordinates services for patients with chronic or behavioral health/chemical dependency needs across the lifespan, and uses the inter‑disciplinary team structure to build longitudinal care plans and meet patient goals.

Nature and Scope

The Care Coordinator will coordinate all components of Care Coordination services throughout the continuum of care. This includes organizing patient care activities, sharing information among all participants, and ensuring that patient needs and preferences are known ahead of time and communicated at the right moment. The Coordinator will follow Renown policies and procedures, the Care Coordination Model of Care, and CMSA guidance.

The role offers cross‑training opportunities within the department and may cover for vacations, illness, or vacancies.

Responsibilities
  • Strong interpersonal communication skills—both verbal and written.
  • Remain productive and provide help and support to team members.
  • Collaborate with patients, families, providers, and team members to develop a patient‑centered Plan of Care and support self‑management goals.
  • Coordinate alternative community resources, including Home Health Care, REMSA, Durable Medical Equipment, Social Determinants, and community partners to promote a safe and patient‑aligned environment.
  • Facilitate problem‑solving with patients, families, providers, and other health care professionals to resolve care issues effectively.
  • Understand the Care Coordination process (Assessment, Planning, Goal Setting, Intervention, Evaluation) and apply it to individual patient needs to promote positive outcomes.
  • Help with transitions of care and organize medical information.
  • Maintain knowledge of applicable regulatory requirements and community resources.
  • Participate in continuous quality improvement processes.
  • Consistent with Renown Health’s corporate culture.
  • Initiate, update, and revise Assessments, Patient Outreach Encounter documentation, and Longitudinal Plans of Care within the Health Planet module in Epic.
  • Document grievances, complaints, or compliments in the MIDAS system.

This position may be patient facing— in person, e‑visits, home visits, virtual or telephonic.

May be responsible for other duties as assigned.

Minimum Qualifications

Education: Must have working‑level knowledge of English, including reading, writing, and speaking. Appropriate education to obtain and maintain Registered Nursing licensure in the State of Nevada.

Experience: One year as an RN preferred. Applicants with Care Management or Home Health experience preferred.

License(s): Valid State of Nevada or California driver’s license and ability to pass Renown Health’s DMV Report criteria (excludes 200373). Ability to obtain and maintain a State of Nevada RN license at time of hire. Fingerprint verification required per Nevada Division of Public and Behavioral Health background checks, with renewals every 5 years.

Certification(s): Utilization or Case Management Certification is desirable.

Computer / Typing: Must possess or be able to obtain within 90 days the computer skills needed for online learning, policy access, benefits enrollment, and other job‑specific competencies.
