RN Case Manager – Home Health – Flexible Hours

Comprehensive Home Health Solutions is seeking an experienced Registered Nurse (RN) to provide skilled nursing care in patients’ homes across Northern Nevada, including Reno, Sparks, and Carson City. This role is available as full‑time and part‑time, with flexible scheduling.

Locations: Northern Nevada (Reno, Sparks, Carson City, surrounding areas)

• Provide direct, one‑on‑one skilled nursing care to patients in their homes.
• Complete comprehensive OASIS assessments and develop individualized care plans.
• Monitor patient progress, communicate changes, and coordinate care with the interdisciplinary team.
• Educate patients and families on disease processes, medications, and self‑care strategies.
• Ensure documentation is timely, accurate, and fully compliant with Medicare and agency standards.
• Collaborate closely with physicians, therapists, and home health aides to ensure seamless, high‑quality care.
• Participate in case conferences and contribute to continuous quality improvement.

• Active Registered Nurse (RN) license in the state of Nevada.
• Minimum of 4 years of total nursing experience.
• At least 2 years of home health experience in a Medicare‑certified home health agency.
• Proficient with OASIS assessments and Medicare documentation requirements.
• Strong clinical judgment, communication skills, and attention to detail.
• Reliable transportation, valid driver’s license, and current auto insurance.
• Familiarity with EMR systems such as Axxess is a plus.
