Nurse Practitioner or Physician Assistant Primary Care​/Family Medicine

Providence Medical Group (PMG) is seeking an experienced nurse practitioner or physician associate to join dedicated Primary Care providers at PMG Sherwood Family Medicine clinic. You'll be part of a collaborative, value-based, and patient-centered medical home made up of physicians, advanced practice providers, behavioral health providers, clinical pharmacist, case management, and an experienced clinical support team. Located 15 miles from downtown, this family-friendly suburb in the Tualatin Valley offers small town charm, fantastic schools, several protected parks and nature preserves, a short drive to the wineries of Newberg, while still easily accessible to all the city amenities available throughout Portland, Oregon.

• Full-time (up to 1.0 FTE)
• 4 to 5 days per week schedule, Monday to Friday
• Paneled, team-based care with direct and indirect patient care (In Basket management)
• 2-years of post-graduate professional experience in a primary care setting strongly preferred
