Full‑time physician available immediately to provide primary care services to frail participants of the Neighborhood PACE program.

• Provide geriatric primary care in PACE clinics, at home, and in assisted living/nursing facilities.

• Certification in Adult or Family Medicine (Geriatrics certification desirable).
• Significant experience practicing with frail adults.
• Spanish proficiency preferred.
