RN Case Manager
May Institute – Registered Nurse Case Manager – Residential Services
OverviewThe RN Case Manager provides clinical oversight and case management for individuals living in residential programs serving adults with developmental and intellectual disabilities, many of whom have complex medical and behavioral needs. The RN CM ensures that nursing services are delivered in accordance with regulatory standards, agency policies, and individualized care plans. This position collaborates closely with clinicians, residential management, direct care staff, families, physicians, and external agencies to promote optimal health outcomes and quality of life for the individuals supported.
Key Responsibilities- Clinical Oversight & Care Coordination – Provide ongoing clinical assessment of individuals in assigned homes, review and update physician orders, MARs, and treatment sheets monthly and as changes occur, monitor acute changes, coordinate provider communication, oversee implementation of care plans, collaborate with guardians, residential managers, and healthcare providers.
- Staff Support & Training – Train and mentor direct support staff on clinical procedures, symptom recognition, documentation standards, emergency response, cover nursing call‑outs when necessary, provide coverage during call‑outs as needed, conduct medication administration competency checks, ensure staff maintain MAP certification compliance, provide ongoing education in infection control, aspiration prevention, bowel protocols, diabetes management, and other individualized plans.
- Documentation & Compliance – Maintain timely and accurate nursing documentation within agency and DDS guidelines, participate in internal audits, QA reviews, investigations, ensure compliance with DDS, DPH, and other regulatory requirements, review incident reports for clinical follow‑up and ensure corrective actions are implemented.
- Team Collaboration – Participate in interdisciplinary team meetings, contribute clinical input for ISP development and review, work collaboratively with residential managers to promote a culture of communication, accountability, and proactive care, communicate effectively with physicians, hospitals, and external providers regarding treatment plans and transitions of care.
- Current MA Registered Nurse (RN) license in good standing.
- Minimum of two years of experience in developmental disabilities, long‑term care, or community nursing.
- Strong clinical assessment, organization, and communication skills.
- Ability to manage multiple priorities across several homes and respond flexibly to emergent needs.
- Map Trainer Certified or willing to obtain MAP Trainer certification.
- Valid driver’s license and reliable transportation required.
$90,000 – $97,000 per year.
