Mountain View Hospital is looking for a HIM Clerk to join our team!
Job SummaryReviews medical records after discharge for completeness in compliance with the hospital's bylaws, federal and state requirements. Ensures that patient’s abstracts are correctly entered into the chart deficiency system. Monitors accounts being admitted daily for duplication of medical record numbers and checks patient discharge status when assembling the chart. Ensures that all transcribed reports are filed in a timely manner in the charts.
Communicates with physicians about delinquent reports. Accountable for the accurate transcription of dictated reports. Ensures completion of incomplete medical records within the time frame specified by the medical staff rules and regulations. Is familiar with state guidelines for processing of charts (History and Physical, Operative reports). Also knows Medical Record content, maintenance and retention of records.

Note:
Benefits are based on eligibility according to full-time, part-time or PRN status classification.
- Collect discharged patient charts from the patient care units daily.
- Collect loose fillings for discharged charts and file in proper charts.
- Assemble discharged patient charts according to the assembly list and place in proper folder by medical record number.
- Analyze medical records of discharged patients for completeness and accuracy per department and hospital policies.
- Assist in retrieving charts for physician completion.
- Answer incoming calls to the department professionally and promptly.
- Forward analyzed charts to the coder and pull delinquency charts according to deficiency list.
- File or scan charts back to main file and assign medical record numbers for new charts.
- Communicate clearly with supervisor, nurse managers, co-workers and physicians.
- Handle correspondence from insurance companies and attorneys; obtain requested medical records to fill requests.
- Complete annual education requirements and maintain workload, processing a minimum of 25 charts per day.
- Report to work on time, review charts for diagnoses/procedures, and perform quality reviews on records scanned in Evident.
- Reconcile master list with in-house list daily; verify admit and discharge dates in the mainframe.
- Request charts daily from storage for next-day procedures; file and clear charts in the dictation room by end of shift.
- Maintain knowledge of hospital policies and services; demonstrate flexibility, organization, and ability to work under stress.
- Maintain patient confidentiality and treat patients and families with respect and dignity.
- Participate in educational programs, in-service meetings, and complete annual educational requirements.
- Assist with other duties as assigned.
Education/Certification: High school diploma or general education degree (GED).
Experience: One to three months related experience and/or training; or equivalent combination of education and experience.
Equipment/Technology: Ability to use hospital information system and operate basic computer functions and software.
Language/Communication: Ability to read and interpret documents such as safety rules and procedure manuals; ability to write routine reports and correspondence; ability to speak effectively before groups of customers or employees.
Mathematical

Ability to perform basic arithmetic in all units of measure, using whole numbers, fractions, and decimals.
