Nurse Practitioner, Primary Care

Position Overview

Nurse Practitioner, Primary Care for Reynoldsburg at Trinity Health (Mount Carmel). Full‑time, day shift. This is an Internal Medicine / Primary Care opportunity preferred for APRN experience in an outpatient setting. The office is busy and requires collaboration with the administrative team and physicians.

• Manages the care and treatment of patients within the professional scope of practice and current standards and protocols.
• Develops a comprehensive plan of care with the patient/family consistent with established guidelines.
• Practices within the guidelines of a Standard Care Arrangement with a collaborating physician.
• Initiates and leads performance improvement activities focused on achieving top quartile on clinical indicators.

• Master’s degree in Science, Nursing, or a related specialty and completion of an Advanced Practitioner Program.
• Current RN license and Nurse Practitioner license in Ohio; certification as an Advanced Practice Nurse in a designated specialty.
• BLS provider certification.
• DEA license.
• Strong clinical assessment and analytical skills.
• Self‑directed professional with strong organizational skills.
