Job Description Summary
This is a full‑time evening shift position (1:00 pm – 1:30 pm) that floats to New Albany, Obetz, and Reynoldsburg. The LPN provides direct nursing care to patients and families across the health‑illness continuum while collaborating with the health‑care team. The LPN delegates nursing tasks to unlicensed assistive personnel (UAPs) in accordance with the Ohio Nurse Practice Act. This role serves as a resource for other associates and participates in process improvement activities;
it has no supervisory responsibilities.
Responsibilities and Duties
- Perform initial, ongoing, and functional health status assessments (25%). Collect and document subjective and objective data, review team findings, communicate assessment results via rounds, huddles, reports, and care conferences; identify nursing diagnoses and collaborate on care plans.
- Document planned nursing interventions based on diagnoses and collaborative problems; initiate and coordinate care plans, promote cost‑effective care, and achieve expected outcomes (25%).
- Evaluate and document responses to interventions, revise care plans as needed (20%).
- Delegate tasks per state statutes, review delegated work, coordinate continuity of care, and coach other employees (20%).
- Participate in process‑improvement activities, advocate for patients, attend committees, and manage resources to maintain a safe environment (10%).
Minimum Qualifications
- High School diploma or GED (required).
- BLS certification – American Heart Association.
- Licensed Practical Nurse (LPN) license from the State of Ohio.
- Successfully completed Ohio Board of Nursing approved medication course.
Additional Requirements
- Must hold a current State of Ohio license.
- Basic Life Support Provider.
