Kenniscentrum Sociaal Domein in Rhenen zoekt een ervaren Wmo Consulent GGZ & Bemoeizorg. Je begeleidt inwoners met meervoudige problematiek en GGZ-vraagstukken, richt je op complexe casussen en werk samen met diverse partners.
Je hebt een hbo-opleiding en minimaal 4 jaar ervaring in de functie. Wij bieden een uitdagende baan met mogelijkheden voor ontwikkeling en een salaris volgens de gemeente CAO.
