An exciting opportunity has arisen for an Enhancing Lives Co-ordinator, within the Harm Reduction Team of Substance Misuse Services. The role will cover both Conwy and Denbighshire.

To oversee the management of the complex cases of people that the Enhancing Lives Team are working to support. Working directly with the caseload of people and the wider Enhancing Lives Team to develop and support the planning of what interventions are needed from the various parts of the system.

Main duties of the job
  • To be a senior member of the Enhancing Lives Team.
  • To work as the Co-ordinator for the Enhancing Lives Team; leading and supervising the team in taking a joined up approach to managing a caseload of people who are experiencing complex needs in relation to issues including substance use, homelessness, mental health and offending behaviour, which is often underpinned by trauma.
  • To lead and manage the implementation of health and social interventions to achieve the outcomes of engaging/re-engaging individuals that are experiencing complex needs, providing an integrated approach for service access.

The ability to speak Welsh is desirable for this role;

Person Specification Qualifications And Knowledge
  • This post requires a confident, dynamic and experienced individual, with relevant experience within partnership management,
  • Relevant first degree or evidence of working practice/education to first degree level
  • Post graduate qualification in relevant field or equivalent level of experience
  • Evidence of continued professional development, with courses relevant to area of work
  • Sound knowledge of current issues and legislation in one or more of the following areas substance misuse, mental health, homelessness and criminal justice
  • An understanding of a range of services, including substance misuse, mental health, housing, criminal justice primary care, secondary care and third sector agencies.
  • An understanding of the laws relating to confidentially, patient consent and data protection
  • A clear understanding of governance and evidence based practice
Experience
  • Experience in one or more of the following areas; substance misuse, housing, mental health or criminal justice.
  • Understanding of the development of policy and/or guidelines
  • Evidence of experience of multi-disciplinary and multi
    -agency working
  • Experience of developing, and evaluating project delivery
  • Experience of delivering new service developments
  • Experience of developing strategies guidelines and policies
  • Experience of appraisal and supervision of staff
Aptitude and Abilities
  • Exceptional interpersonal and communication skills
  • Ability to assess and deliver, evaluate and benchmark quality care
