community nursery nurse vacancy: band 4 permanent part time.

betsi cadwaladr university health board has an opportunity for an enthusiastic and innovative community nursery nurse to join our flying start health visiting service denbighshire, north wales. We are looking for a self-motivated team player who has excellent communication, organisational and interpersonal skills.

the post holder will facilitate one to one, group and community work that aims to address health inequalities in partnership with the wider multi-skilled team. He/she will take a key role in delivering parent support, health promotion and programmes of care. He/she will provide a service to families as identified in the needs assessment carried out by the health visitor and undertake planned packages of support through implementing and evaluating planned programmes of care.

In addition, the post holder will contribute to the public health role in line with government public health initiatives.

the health board offers an induction course and ongoing clinical supervision and support from its safeguarding children team and opportunities for in house training.

the ability to speak welsh is desirable for this post; welsh and/or english speakers are equally welcome to apply.

main duties of the job

support health visitors in delivering health promotion within home, clinic and other community based settings as agreed.

contribute to the implementation of the delivery of the core programme as per health board service level agreement and directed by the health visitor.

following initial assessment by hv, carry out delegated programmes of care or follow up health issues at home or in clinic.

undertake screening and/or other health related and developmental assessments as and when directed by the health visitor.

refer to appropriate agencies in collaboration with the health visitor.

ensure the delivery of evidence-based practice in order to facilitate effective care. Includes issues relating to health and well being e.g. Diet; safety; dental care etc.

work with parents to increase their knowledge and skills to enable them to identify and meet their own and children's health needs under the supervision of the health visitor.

work in partnership with service users and providers; attend client participation groups, social services meetings as directed by the health visitor.

work in partnership with the community, statutory and voluntary agencies in the provision and development of individual and community initiatives as directed by the health visitor.

undertake drop in clinics.

facilitate groups, eg parent and baby groups - as directed by the health visitor.

undertake baby massage sessions.

help to organise and lead parenting programmes.

to assist the caseload holder in the management and delivery of care to clients autonomously. Duties may include:

building on existing assessment of families needs and deliver packages of planned care with no direct supervision.

to provide and receive complex and sensitive child related information from health and social care staff.

to decline referral if not appropriate which requires excellent negotiation skills and appropriate sign posting.

support to clients with more complex/additional needs.

to demonstrate ability for motivation and behaviour change with children and families enabling self-efficacy, resilience and strength based relationship.

to recognise and report any health and safety or risk issues.

to write records in a concise, accurate and timely manner in line with gdpr guidance and bcu local polices.
