This post is fixed term/secondment for 12 months due to - meet the needs of the service

An exciting opportunity has arisen for a Band 6 nurse to join our renal specialist nurse team.

The main focus of the role is planning, assessing and maintenance of renal patients and requires a quality improvement focus in addition to the expected skills and knowledge set out in the Job Description.

The post holder will work as part of the renal specialist nurse team, in the delivery of service in North Wales and a part of a Multi-Disciplinary/Multi Service team to ensure a consistent and integrated approach to patient care.

Clinical audit is core to the monitoring of service standards and delivery. The post holder will be proactive in meeting the educational and training needs of patient and nursing staff.

• To support the BCUHB in increasing the number of renal/pancreas transplant patients, and living donor transplants.
• To provide specialist nursing expertise, knowledge and skills in the clinical practice of transplantation nursing care.
• To be an agent for change to emphasise patient empowerment and self-care within defined clinical and organisational boundaries.
• To enhance education through the teaching of patients, families, and colleagues in order to enhance clinical practice and improve patient care.
• To provide a consistent approach and share best practice across BCUHB.

The ability to speak Welsh is desirable for this post;
