• Perform clinical assessment and identify patient needs.
• Document, prepare, and complete patient Plan of Care; track progress and revise as needed.
• Meet mandatory continuing education requirements.
• Plan and provide appropriate treatment for neuro-muscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
• Assist and instruct patients and families in transfer techniques appropriate to patient abilities; instruct on use of equipment (walkers, wheelchairs, canes, etc.) safely.
• Utilize appropriate therapy equipment specific to patient diagnosis and needs.
• Promote company mission and administrative policies to ensure quality of care.

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program.
• Current Texas State License as a Physical Therapist.
• Current Texas Driver's License and reliable transportation.
• One year of experience as a Physical Therapist in acute care or rehabilitation setting; pediatric experience required.
• Effective interpersonal and communication skills.
• Knowledge of durable medical equipment.
• Ability to work a flexible schedule and travel locally.
• Able to work in patient's home in various conditions.
• Excellent organizational and time management skills.
  
  Proficient in Microsoft Office.
• Maintain high confidentiality.
• Proof of current CPR certification.
• Ability to do heavy lifting, bending, pulling, pushing, and standing; prolonged standing and walking required.
• Ability to work under some stressful conditions to meet deadlines and patient needs.
