• Perform clinical assessment and identify patient needs.
• Document, prepare, and complete patient Plan of Care; track progress and revise as needed.
• Meet mandatory continuing education requirements.
• Apply infection control and universal precautions to prevent contamination and transmission.
• Provide appropriate plans, treatments, and evaluations for speech, language, cognitive, and swallowing disorders with relevant tests and measurements; maintain pertinent records.
• Communicate and educate families concerning patient deficits, treatment facilitation, and discharge plans.
• Promote company mission and administrative policies to ensure quality of care.

• Current Texas State License as Speech Language Pathologist.
• Current Texas Driver's License and reliable transportation.
• One year of experience as a Speech Language Pathologist in an acute care or rehabilitation setting; pediatric experience required.
• Effective interpersonal relations and communication skills.
• Knowledge of durable medical equipment.
• Ability to work in a patient's home in various conditions.
• Excellent organizational and time management skills.
• Proficient in Microsoft Office.
• Maintain a high level of confidentiality.
• Proof of current CPR certification.
• Ability to lift, bend, pull, push, and stand; prolonged standing and walking required.
• Work well under stressful conditions to meet deadlines and patient/family needs.
