• Provides occupational therapy services to patients by assisting in assessment, treatment, program planning, and implementation.
• Documents treatment plans, monitors patient response, and coordinates with the supervising OT and other team members.
• Conducts screenings and evaluates residents at appropriate intervals, including self‑care, functional mobility, and cognition.
• Develops patient goal setting, short‑ and long‑term treatment plans, and provides individual treatment as outlined.
• Assists with the selection and adaptation of occupational therapy techniques/media and devices.
• Collaborates with patients, families, and staff on education, device training, and documentation.
• Adheres to Northwoods Rehab procedures for documentation, safety, infection control, and OSHA standards.
• Maintains professional standards, confidentiality, and a positive working relationship with staff, residents, and family.
• Participates in quality assurance and other duties as assigned.

• Associate’s degree in Occupational Therapy from an accredited program.
• Current state licensure or license‑eligible.
• Assumes responsibility for ongoing continuing education and professional development.
• Ability to meet medium‑level physical demands (e.g., lifting up to 50 lbs occasionally, standing, walking, bending).

Reports to the supervising Occupational Therapist and Director of Therapy/Rehab.

• Medium work: exerting up to 50 lbs occasionally, up to 25 lbs frequently, 10 lbs constantly.
• Standing, walking, bending, occasional push/pull, overhead reach, etc.
• Average workday up to 8 hours. May encounter unpredictable patient/resident behaviors.
