Job Description Summary
The Float Nurse is a vital member of the hospice interdisciplinary team who provides flexible clinical support across assigned service areas. This position does not carry a dedicated patient caseload; instead, the Float Nurse assists with patient visits, coverage needs, and care coordination to support team workflow, continuity of care, and quality patient outcomes. The Float Nurse delivers compassionate, patient-centered hospice care while collaborating closely with clinical and interdisciplinary team members.
Essential Job Functions and Responsibilities:
- Complete baseline patient visits as assigned;daily assignments and priorities are determined by clinical leadership based on patient needs, staffing, geography, and organizational priorities.
- Provide hospice nursing care through routine, comprehensive, face-to-face, and symptom follow-up visits in patient homes, facilities, and other care settings.
- Perform comprehensive assessments and ongoing evaluations of patient condition, needs, symptoms, and response to treatment.
- May provide coverage for admissions, urgent/PRN visits, symptom management, routine visits, and other nursing needs as assigned across Heartlinks service areas.
- Identify changes in patient status and communicate pertinent findings, recommendations, and care needs to the interdisciplinary team.
- Clear handoff back to the RN Case Manager/clinical team, particularly for changes in condition, medication concerns, safety issues, or follow-up needs.
- Participate actively in interdisciplinary team (IDT) meetings and contribute to care planning and coordination.
- Provide education and support to patients, families, and caregivers.
- Support team workflow by providing visit coverage and clinical assistance as needed.
- Complete timely and accurate clinical documentation.
- Maintain compliance with hospice regulations, professional standards, and Heartlinks Hospice policies.
- Promote compassionate, patient-centered hospice care.
Position
Qualifications:
- Graduate of an accredited School of Nursing.
- Current Registered Nurse license in the applicable state.
- Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing, is preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently while maintaining effective collaboration with interdisciplinary team members.
- Strong assessment and clinical decision-making skills.
