Services you will provide
  • Individualized pharmacotherapy
  • Counselling patients about their medication therapy
  • Work in collaboration with physicians, and other health care providers to develop, implement and monitor therapeutic plans
  • Assist in the development of drug therapy guidelines for individual disease conditions
  • Prepare and submit case studies and articles for professional journals
  • Act as department resource person within your assigned area
  • Participate in medication use evaluations, quality improvement initiatives and clinical drug trials
  • Provide mentorship and supervision to pharmacy students, residents and new staff
  • Ensure prescriptions are validated and dispensed accurately; resolve medication safety concerns expediently; document professional practice activities proficiently

Responsibilities
  • Consult with physicians, nurses and other health care providers to develop and discuss therapeutic plans based on chart review and assessment of clinical responses. Ensure appropriate documentation is maintained in the patient health record.
  • Individualize patient medication therapy by reviewing literature, patient charts, lab results, and meeting with the patient and health care team to assess clinical response and modify therapeutic plans.
  • Provide patient medication counselling regarding drug therapy and demonstrate procedures, techniques and use of equipment.
  • Assist the health care team in developing drug therapy guidelines for disease conditions by reviewing literature and monitoring guideline effectiveness.
  • Participate in ward rounds, program team meetings and provide pharmacy services at the unit level to enhance patient care.
  • Act as department resource person in your area of responsibility, supporting staff with detailed drug information.
  • Contribute case studies and review articles to professional journals.
  • Provide instruction and/or supervision to pharmacy students and residents.
  • Participate in quality improvement activities, including audits and recommending changes.
  • Perform drug distribution duties, interpret prescriptions, and dispense medications using the computerized system in accordance with policies and standards.
  • Review prescriptions for therapeutic suitability, interpreting lab data and patient demographics.
  • Communicate with prescribers for clarification and problem resolution; document outcomes in the health care record.
  • Ensure medications are efficiently and accurately dispensed, compounded, labeled, and record keeping is completed.
  • Collect data for medication use evaluation projects and assist with inventory management.
  • Participate in clinical drug trials and document adverse reaction cases.
  • Implement medication safety initiatives and quality control activities.
  • Attend staff meetings, committees, and other programs as required.
