Position: Full-time (1.0 FTE), 37.5 hours/week
Schedule: 5 days per week with evenings and some weekends as required
Location: On-site at the hospice

Compensation Highlights: Salary range $28.20 hourly to $32.30 hourly 15 days’ vacation, 5 personal days, competitive health and dental benefits (after probation); participation in HOOPP (Health Care of Ontario Pension Plan).

The opportunity

Reporting to the Director, Community Care, the Community Care Navigator (Navigator) is responsible for general administration, intake coordination and timely and accurate responses to inquiries and referrals. Using a person-centred, equity-informed approach, the Navigator conducts initial registration and screening to understand psychosocial and bereavement needs, determines service suitability, and facilitates access to appropriate internal and external supports. The role supports individuals and families in navigating complex health systems and works to reduce barriers to care.

The Navigator collaborates closely with DLH program staff and Volunteer Management Team to coordinate and integrate volunteer programming.

Key responsibilities

Intake and program administration
  • Ensure accurate and timely processing of new referrals to DLH including data entry into the Electronic Medical Record (EMR) and any additional data collection tools, communication with referrers, and supporting individuals through self-referrals
  • Being the first point of contact for inquiries from health care professionals
  • Being the first point to the general public for information, navigation, and informal emotional support.
  • Conduct registration and intake screening of community referrals (palliative, caregiver, bereavement).
  • Provide navigational support to health and social services outside of DLH
  • Provide general administrative support for all DLH community team programs, including supporting mailouts, data collection, reporting, and evaluation
  • Respond to ad hoc in-person needs for those come to the DLH building
  • Participate in the Hospice Helpline on-call rotation (after hours)
  • Support the delivery of DLH education and engagement events
Volunteer coordination and support
  • Coordinate the community volunteer program for in-home visiting volunteers and bereavement volunteers
  • Coordinate and support the delivery of support groups led by volunteers
  • Provide support to the Volunteer Management Team regarding recruitment, training, performance, engagement and general management of Community Team volunteers
Qualifications and experience
  • Registered Social Worker (BSW), or equivalent work experience as recognized by the Hospice
  • An active member in good standing with the applicable regulatory body/college
  • Minimum of two (2) years experience working with people with a life limiting illness, caregivers and those who are bereaved
  • Demonstrated proficiency in triaging, problem solving and working with ambiguity
  • Expert knowledge of community resources relevant to palliative care, grief and trauma
  • Experience navigating multiple electronic systems and electronic health records
  • Committed to protecting the privacy and confidentiality of persons served
  • Demonstrated excellent interpersonal and communication skills
  • Effective and efficient organizational and time management capabilities
  • Prior experience in hospice, health care or non-profit organizations is an asset
