In this role, you will utilize your expertise to perform thorough psychosocial assessments and provide both individual and group counseling. Collaboration is key, as you will coordinate with healthcare professionals to build comprehensive care plans that cater to the needs of your clients. Your influence will also extend to community resource development, enhancing support systems for those in need.
Key Responsibilities:
• Execute functional assessments and care planning for clients
• Facilitate individual and family therapy sessions
• Document and update client records effectively
• Liaise with interdisciplinary team members for enhanced care
• Identify community resources to support client needs
Requirements:
• Master's degree in Social Work from an accredited program
• Minimum three years in case management with complex cases
• Registered with the British Columbia College of Social Workers
• BC Driver's License for local travel
• Proven ability to apply best practices in case management
