This position demands a dynamic approach to client and family engagement. You will perform psychosocial assessments and provide tailored counseling, collaborating with a multidisciplinary team to ensure continuity and effectiveness of care plans.
Your role also includes identifying vital community resources to bolster support structures.
Key Responsibilities:
• Conduct comprehensive psychosocial and functional assessments
• Provide individual and group counseling tailored to client needs
• Maintain thorough and accurate client case documentation
• Work with healthcare professionals on interdisciplinary teams
• Develop and leverage community resources for client benefit
Requirements:
• Master’s degree in Social Work from an accredited institution
• Three years of relevant case management experience with various client demographics
• Active registration with the British Columbia College of Social Workers
• Valid Driver's License for travel in local areas
• Strong foundation in chronic disease and case management principles
