Support diverse communities in Thunder Bay as a System Navigator with Nor West Community Health Centres. This full-time role focuses on advocacy and care coordination for marginalized individuals.
As a System Navigator, you will be an essential part of an interdisciplinary team committed to health equity and client-centered care. This position engages with Black youth and families, providing culturally responsive navigation and integrated services across community settings. You will facilitate access for individuals facing healthcare barriers, including mental health challenges and systemic discrimination.
Key Responsibilities:
• Provide client-centered navigation and advocacy services
• Engage in assessment and care planning with clients
• Connect clients to culturally relevant health resources
• Collaborate with community partners to enhance support
• Promote health equity and family empowerment in the community
Requirements:
• Bachelor’s degree in Social Work, Nursing, or related field
• 2-3 years' experience in system navigation and community outreach
• Strong understanding of social determinants of health
• Valid driver’s license and reliable vehicle access
• Flexibility for occasional evening and weekend work
