Part‑time Registered Nurse Case Manager
  • Anticipated Territory: Richmond Area
  • Schedule: Must be available full days every Saturday and Sunday (4 SOCs/day). Also willing to consider full time:
    Saturday and Sunday plus 3 weekdays.
What you’ll do:
  • Utilize your outstanding clinical skills to complete the initial assessment of patients to determine home‑care needs.
  • Conduct Start of Care (SOC) admissions – OASIS.
  • Perform follow‑ups, evaluation visits as needed in the designated territory.
  • Make home visits to patients in the territory.
  • Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures.
  • Monitor clients’ conditions and report changes to the clinical team.
  • Collaborate with physicians and agency staff to deliver the best care possible.
  • Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
  • Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols.

To Be a Success:
  • Current and unrestricted California RN license.
  • Current BLS/CPR certification.
  • Valid California driver’s license.
  • Experience conducting assessments and start‑of‑care (SOC) – OASIS highly desired.
  • High level of clinical knowledge in home‑health care highly preferred.
  • Experience with high‑acuity patients a plus.
  • Experience working with an interdisciplinary team highly desired.
  • Case‑management fundamentals highly desired.
  • Previous experience with IV medication infusions highly preferred.
  • Proficiency in initiating intravenous (IV) therapy is a plus.
