Job Type: Full‑time travel assignment

• Assess patient conditions and develop individualized care plans.
• Administer medications, treatments, and wound care while monitoring responses.
• Provide patient and caregiver education on medication management, disease progression, and self‑care techniques.
• Monitor for changes in patient status, report concerns, and adjust care plans as needed.
• Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care transitions.
• Document patient care, interventions, and progress accurately in compliance with regulatory standards.
• Ensure compliance with infection control, safety, and home‑care best practices.
• Support families and caregivers, offering guidance and emotional support throughout the care process.
• Maintain communication with case managers and interdisciplinary teams.
• Complete charting and documentation requirements while meeting visit and productivity goals.
• Perform other duties as assigned within the scope of practice.

• One year of recent RN experience, including at least six months in a Home Health setting.
• Proficiency with OASIS documentation.
• Valid RN license in compliance with state regulations.
• BLS certification (AHA/ARC).

• Home Health Nursing Certification (RN‑BC).
• Other relevant certifications or licenses.

• Critical thinking, service excellence, and strong interpersonal communication.
• Proficiency in reading, writing, and speaking English.
• PC and software skills.
• Physical abilities to remain stationary, move equipment (50‑100 lbs), and perform patient transfer tasks.
• Capacity to work with diverse personnel and adapt to irregular schedules.
• Visual acuity and effective communication skills.
