Job Description :
The Chronic Registered Nurse is responsible for delivering and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for a designated group of patients. This role may include delegating tasks to direct patient care staff such as RNs, LVNs/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) healthcare team, the RN actively participates in decision-making, education, leadership, and quality improvement initiatives to improve patient outcomes and facility performance.
Principal Duties and Responsibilities:
Perform essential functions under supervision and guidance from clinical leadership
Conduct ongoing assessments pre-, during, and post-hemodialysis treatment; update treatment plans as necessary
Educate patients and families based on assessed learning needs
Oversee and collaborate in the delivery of safe and effective hemodialysis therapy
Administer medications and document effectiveness and medical justification
Respond to emergency situations as needed
Act as a resource for the healthcare team and assist in training new staff
Ensure proper collection, processing, and shipping of lab samples
Collaborate with the Interdisciplinary Team to identify expected outcomes and update care plans
Educate patients on transplant and treatment modality options
Perform other duties as assigned by the Supervisor
Supervision:
May serve as Team Leader or Nurse in Charge with oversight of Patient Care Technicians/LPNs/LVNs/RNs upon meeting the following requirements:
Successful completion of all required training and education
Minimum 9 months of RN experience
Minimum 3 months of chronic/acute hemodialysis RN experience
Physical Demands and Working Conditions:
Regular lifting and assisting patients; use of equipment or coworker support may be needed
Frequent and prolonged standing, bending, and moving equipment (up to 200 lbs with assistance)
Lifting of chemical/water solutions (up to 30 lbs up to 5 feet high)
Exposure to a healthcare facility environment with moderate noise and air temperature control
Potential exposure to infectious and contagious materials
May require local travel and work in multiple facility locations
Must pass the Ishihara Color Blindness Test; reasonable accommodations will be considered
Education and Licensure Requirements:
Graduate of an accredited School of Nursing
Current state licensure as a Registered Nurse
Current or successfully completed CPR BLS Certification
Must meet practice requirements for the state of employment
Experience and Required Skills:
1+ years of Nephrology Nursing experience as an RN within the last two years
Chronic hemodialysis experience required
