Job Description

Eventus Whole Health delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.

Essential Duties and Responsibilities:

Provide primary-care services following Eventus Whole Health site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine in long-term care settings.

Review and sign MARs, orders, and documentation promptly, including labs, prescriptions, and plans of care, ensuring compliance with facility and company policies.

Manage on-call triage duties as per protocol, adhering to the triage schedule and policies established by Eventus Whole Health.

Complete documentation and submit billing records according to policies, providing copies to the facility within 48 hours of service.

Maintain open communication with facility staff and administration, providing verbal or written feedback as needed.

Meet with the facility care coordinator before and after patient care visits daily.

Adhere to all legal, regulatory, and ethical standards, including HIPAA confidentiality requirements.

Consult with the clinical supervisor before refusing any resident, family, or facility requests regarding resident care.

Qualifications/

Education:

Master's degree in Nursing from an accredited institution and a valid license to practice in your state, or
Graduation from an accredited Physician Assistant program, passing the national certification exam, and licensure in your state. Geriatric, family, or psychiatry specialization is preferred.
