General Position Description

The RN Case Manager performs admissions and administers nursing care for patients of all ages in their place of residence. They coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency and assume responsibility for coordination of care.

Role Expectations

• Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify their physical, psycho‑social, and environmental needs.
• Develop and document the plan of care in correlation with the Assessment.
• Provide nursing care utilizing infection control measures (OSHA).
• Work with the Interdisciplinary Group/Team to ensure patient/family needs are met.
• Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
• Submit all required paperwork timely.
• Participate in hospice-sponsored in‑service programs, maintain or improve competency, and participate in the Hospice’s praia. QAPI program.
• Promote the Hospice’s philosophy and administrative policies.
• Meet productivity standards.
• Perform on‑call responsibilities and provide on‑call services to patients and their families as assigned.
• Provide effective communication to patients, their family members, team members, and other health care professionals.
• Transportation:
  Reliable transportation and valid auto liability insurance.

Requirements

Education and experience:

• Two (2) years experience as a Registered Nurse in a clinical setting and one (1) year full‑time hospice experience.
• Education:
  
  Graduate of96 accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure:
  Current State license as a Registered Nurse; current driver's license.
• Skills:
  
  Nursing skills as defined by generally accepted standards of practice, knowledge of state and federal regulations, and good interpersonal skills.

Physical Requirements

Environmental and working conditions:
Works in patient homes in various conditions; possible exposure to资源网 blood, bodily fluids, and infectious diseases; ability to work a flexible schedule, travel locally, and endure unpleasant weather; PRN emergency calls.

Physical and mental effort:
Prolonged standing and walking required; ability to lift up to 50 lbs and move patients; ability to work under stressful conditions and make quick decisions.

Communication:
Communicate with patients, families, physicians, co‑workers, and visitors to exchange accurate information about patient condition фит health status.

Auditory:
Ability to hear sounds and give full attention to others without interrupting at inappropriate times.

Judgment:
Ability to make independent judgments and decisions using logic and reasoning to evaluate alternative solutions.

Technology:
Ability to view tablet/Electronic Health Record and review, assess, record or type data quickly and accurately.

Transportation:
Ability to travel to patient homes and office/support center locations as needed.
